Are you between the age of 12 and 24 yrs old and living in rural Ottawa? If so, you can fill out an anonymous survey about rural Ottawa youth mental health. The survey takes about 15 minutes to complete, and your answers will help to plan mental health programs and services for rural Ottawa.

At the end of the survey, you can enter a draw to win one of 16 $150 gift cards for a restaurant or shop of your choice given by “Bell Let’s Talk”! Your entry into the draw will not be connected to your answers on the survey.
